Thrive Rewards 2021
HAP Members - Earn a reward of a lower employee contribution rate to your medical coverage or funding to a health savings account for 2022. Plus, get entered into a drawing to win prizes. Both employee and covered spouse must complete the Health Assessment by March 31, 2021 and all remaining program requirements by July 31, 2021 to earn the reward.

Complete all 4 steps to earn the reward
You will earn green check marks as you complete each step. Allow up to two weeks for check marks related to Health Management activities.
Step 1 - Complete your online Health Assessment (HA)
You must complete Step 1 by March 31, 2021 or you will be unable to continue in the 2021 Thrive Rewards Program.
Step 2 – Complete a Well-being activity
All participants must complete this step by July 31, 2021. There are many simple and convenient ways to complete a well-being activity. See iStrive for details.
Step 3 – Be Tobacco Free or Participate in Health Management
To earn credit for this step you must report being Tobacco Free in the Health Assessment OR complete a Health Management activity, including health coaching, Henry Ford Tobacco Treatment or Care Management. For details, log in to your Thrive Rewards program in iStrive. All participants must complete this step by July 31, 2021.
Step 4 – In-Range Results or Participate in Health Management
To earn credit for this step you must have in-range results OR complete a Health Management activity, including health coaching, Henry Ford Tobacco Treatment or Care Management. For details, log in to your Thrive Rewards program in iStrive. All participants must complete this step by July 31, 2021.
If you have lab results in Epic from your in-network physician between 1/1/2018 and 7/31/2021, your values will already be in iStrive. Otherwise data from your Health Assessment will be used to determine whether you meet in-range criteria.
Health Management Activities – Health Coaching and HFA Tobacco Treatment
To sign up for a health coaching session, click here. You must call your coach for your session; the coach will NOT call you.
To sign up for a health coaching session that addresses tobacco use, click here and select Carol as your coach. You must call Carol for your session; she will NOT call you.

Care Management services are intended for Thrive Rewards participants who may benefit from a higher level of support to address their overall health. Care Managers partner with you to provide information on health management techniques, connect you to resources to assist you with reaching your health care goals, and will take the place of health coaching.
Care Management services may include:
- Helping you coordinate care between your doctors and community services
- Helping you better manage stress
- Helping you find ways to get through family or social barriers
- Helping you find transportation to your medical appointments
- Helping you get medications you may have trouble affording
The Care Management program has requirements that each participant is expected to meet, including completing an assessment plus at least one additional session. You are eligible for Care Management for as long as you are engaged in a plan of care.
Logging in to iStrive
HAP members follow these steps to get logged in iStrive: Go to hap.org. Enter your HAP member ID number (found on your insurance card) and password, then click on Log In. If you’re not registered, click on Register Now and follow the prompts to complete your registration. Once you’ve registered, return to hap.org to enter your member ID number and password. Once logged in, click My Health & Wellbeing from the top menu and then iStrive for Better Health. This will take you to the iStrive home page. Click on Rewards at the top of the page to review this year's program and to review your progress.
Non-HAP members follow these steps to get logged in iStrive: Go to webmdhealth.com/HAP. If this is your first year participating, click on "Create Account" and enter your registration ID (registration ID is your full first and last name plust your 8-digit date of birth, example: JosephSample02031982). If you have participated in previous years, enter your user ID and password.
