We are pleased to invite you join the 2011-12 It’s Your Life (IYL) Health Management program. We are looking forward to an exciting year! The following provides information to get you started.
Step 1: Complete Your Online Enrollment Form and Health Risk Appraisal (HRA)
Must be completed by 5 days before your screening date
Your Employee ID for both the enrollment form and HRA is as follows:
- Enter the 3 letter company code, JAX, followed by your employee number. This number can be found on the top of your time sheet.
Read and submit your online enrollment form. Once submitted, you will be directed to take the HRA. Click here to begin.
Step 2: Complete Your Health Screen
You have two options:
1. Have your screening done at your physician’s office and return a Physician Health Screen Form to the It’s Your Life Office by October 31st.
2. Attend an It’s Your Life onsite screening at one of the following locations
- October 17, Courthouse, Emergency Management Conference Room (basement), 7:00 - 10:00 a.m.
- October 19, Tower Building, Commission Chambers, 7:00 - 10:00 a.m.
- October 20, Human Services Bldg., Room 005, 7:00 - 10:00 a.m.
- October 26, Courthouse, Emergency Management Conference Room (basement), 7:00 - 10:00 a.m.
- October 27, Human Services Bldg., Room 005, 7:00 - 10:00 a.m.
- October 28, Tower Building, Commission Chambers, 7:00 - 10:00 a.m.
- November 9, Youth Center, Dining Room, 7:00 - 10:00 a.m.

Printing Your Healthy Lifestyle Profile
Your Healthy Lifestyle Profile will be available for you to print approximately two weeks after you attend your health screen. Please bring a copy to your first health coaching session.
To view and print your profile, click here and login with your name, user ID and password you created. At the end of the profile, you will have the option to print a copy.
