The following is important information on your Health Risk Assessment (HRA):

Step 1: Your Web-Based HRA

• Click here to begin your HRA
• Sign in using your last name.
• Enter your user ID number. This number has been provided to you by the Human Resource Department.
• ID numbers start with RMG.
• Then are followed by the employee number
• Finally, add either a 1 for employees or a 2 for spouses. Ex: John Smith RMG23451 & Mary Smith RMG23452
• Create a password.
• Complete and submit your HRA

Step 2: Complete your Health Screen

It's Your Life will hold health screens on the following dates and locations:

• Kansas - October 6 - 8
• Virginia - October 14-15
• Michigan - October 21 and 23

Step 3: View your results - Available by November 9th

To view and print your profiles, click here and login with your last name, user ID and the password you created. At the end of the profile, you will have the option to print a copy.
