Jackson County Employees
The following is important information on your Health Risk Assessment (HRA):
Step 1: Your web-based HRA
- Click to begin your HRA
- Sign-in using your last name
- Enter your employee ID number beginning with your company code which is JAX. Your employee ID number can be found on the top of your time sheet. Ex: JAX3496
- Create a password
- Complete and submit your HRA
Step 2: Complete your Health Screen
It's Your Life will hold health screens at the following dates and locations:
- October 20, Court House, 7:00 - 10:00 a.m.
- October 21, Human Services Building, 7:00 - 10:00 a.m.
- October 22, Tower Building, 7:00 - 10:00 a.m.
- October 23, Court House, 7:00 - 10:00 a.m.
- October 27, Tower Building, 7:00 - 10:00 a.m.
- October 28, Human Services Building, 7:00 - 10:00 a.m.
- October 29, Youth Center, 7:00 - 10:00 a.m.
- November 4, Tower Building, 7:00 - 10:00 a.m.

Printing Your Healthy Lifesyle Profile
Your healthy lifestyle profile will be available for you to print approximately two weeks after you attend your health screen.
To view and print your profile, click here and login with your name, user ID and password you created. At the end of the profile, you will have the option to print a copy.
