It’s Your Life Health Management Program
Step 1: Complete Your Online Health Risk Appraisal (HRA)
Step 2: Provide Health Screening Results (Two Options)
Option One: Provide health screening results from your personal physician. Screening data from January 1, 2010 or later is approved to be used
Take the Physician Health Screen Form to your doctor to have the listed tests completed. This form is due June 1st.
In order for your Health Risk Appraisal to be processed, you will need to mail BOTH the Physician Health Screen Form AND a signed copy of the It’s Your Life Enrollment Form/Waiver
to the It's Your Life office by June 1st.
Option Two: Schedule an appointment with the It’s Your Life Program for a health screening at the It’s Your Life office at One Jackson Square in downtown Jackson. There is a $45 fee that
will be payable to your Human Resource Office.
To schedule your appointment, call the It’s Your Life Office at (517) 780-7306. Please identify yourself as a Jackson Community College employee.
Step 3: View your results
- Your results will be available in mid-June after all paperwork has been received.
- Click here to view and print your profile
- Login with your last name, Employee ID and the password you created.
- At the end of the profile, you will have the option to print a copy.
- Please print a copy and bring it to your first health coaching session.
Step 4: Complete your Health Coaching Sessions
Each participant will have two 15-20 minute health coaching session to review your results, set a Personal Wellness Plan and review your progress. The first session will be in mid July with a follow-up session in February.
