The following is important information on your Health Risk Assessment (HRA):
Step 1: complete Your On-line HRA
- Click here to begin your HRA
- Sign in using your last name
- For employees: Enter the company code, GLI, followed by your employee ID letters.
- Example: John Jacob Thompson - GLIJJT
- For spouses: Enter the company code, GLI, followed by your first and last initial, then followed by the letter S.
- Example: Mary Thompson - GLIMTS
- Enter your password from the last time you did your on-line HRA or create your password if this is your first on-line HRA
- Complete and submit your HRA
Step 2: complete Your Health Screen
Health screenings will be conducted on-site at Great Lakes Industry on the following dates:
- Tuesday, February 16, 6:30 – 10:30 a.m.
- Wednesday, February 17, 6:30 – 10:30 a.m.
- Wednesday, February 17, 2:00 – 4:30 p.m.
- Tuesday, February 23, 6:30 – 10:30 a.m.
Step 3: View your results four Weeks after your Health Screen
- Click here to view and print your profile
- Login with your last name, Employee ID and the password you created.
- At the end of the profile, you will have the option to print a copy.
- Please print a copy and bring it to your next health coaching session.
