Commonwealth Health Management Program

The following is important information on your Health Risk Assessment (HRA):

Step 1: Your Web-Based HRA

• Click here to begin your HRA
• Sign in using your last name
• The User ID for Commonwealth employees and spouses is as follows:
• User ID starts with CAI, then is followed by the employee ID letters. (Spouses will use the last 4 digits of their Social Security number). Ex: CAI3456
• Create a password
• Complete and submit your HRA

Step 2: Complete your Health Screen

Health screenings will be conducted on-site on the following dates:

• Wednesday, March 25: 6:30 - 10:30 a.m.
• Tuesday, March 31: 6:30 - 10:30 a.m.

Step 3: View your results four Weeks after your Health Screen

To view and print your profiles, click here and login with your last name, Employee ID and the password you created. At the end of the profile, you will have the option to print a copy. Please print a copy and bring it to your next health coaching session.
