Health Management Program
The following is important information regarding your Health Risk Appraisal (HRA):
Step 1: Complete and Return the Physician Health Screen Form
- Take advantage of your health plan and schedule a preventive care visit with your physician. Current information on your height, weight, blood pressure and blood screening results will be needed to complete this form.
- You do not need to have another physical if you have had one September 1, 2009 or later. You can bring the form to your doctor and have them complete it with those results.
- The deadline for this form to be received by the It’s Your Life Office is March 31, 2010.
Step 2: Complete Your Health Risk Appraisal (HRA)
- Click here to begin your HRA
- Sign in using your last name
- Enter your Employee ID
- Your employee ID number begins with your company code which is JAC and is followed by your City of Jackson employee number. Your employee ID number can be found on your paycheck and time sheet.
- Create a password
- Complete and submit your HRA.
- The deadline to complete your HRA is April 30, 2010.
Step 3: View and print your results
- Your report will be available by May 15, 2010.
- Click here to view and print your profile.
- Login with your last name, Employee ID and the password you created.
- At the end of the profile, you will have the option to print a copy.
- Please print a copy and bring it to your first health coaching session.
