New Employees Information for Enrollment in It's Your Life
Steps for enrollment:
1. Complete on-line Health Risk Appraisal (HRA)
If you or your spouse cannot access the HRA, contact It's Your Life for assistance X-7495.
2. Complete your Health Screen (including labs): (two options)
- Attend a health screen on-site at Allegiance Health, Department of Prevention and Community Health. Call (517) 841-7495 to schedule an appointment.
- Please make sure you do not eat or drink anything 10-12 hours prior to your scheduled appointment so that you can have your labs drawn. Labs are required for all participants for enrollment this year.
OR
- Ask your physician’s office to complete the Physician Health Screen form included in this folder. If you choose this option, please make sure all labs and measurements were completed within the last 3 months.
3. Complete Enrollment form
For benefit eligible employees, successful enrollment must be complete 60 days from your date of hire.
