Welcome! We’re looking forward to a great year with the 2011-2012 It’s Your Life program.
It’s Your Life Participants need to complete the online enrollment form and Health Risk Appraisal (HRA) by Wednesday, November 30th.
Onsite health screenings will begin Monday, December 12th. You do not need to schedule a health screening if a Physician Health Screen form is submitted by November 30th.

Your unique ID for both the enrollment form and HRA is as follows:
- The ID starts with GLE
- Then is followed by your assigned It’s Your Life number
Example: GLE123
