Welcome! We’re looking forward to a great year with the It’s Your Life program.
All It’s Your Life participants need to complete the online enrollment form and Health Risk Appraisal (HRA) by Saturday, March 31st.
Please read the following directions then click on the link below to begin.
Your ID for both the enrollment form and HRA is as follows:
Enter the company code, C2AE, followed by the last 4 digits of your social security number.
Step 1: Read and submit your online enrollment form.
